Our Community Care team continues to provide intensive support services and medical care to people living with HIV/AIDS. We work to reach out into the community to help to increase our care to the people who need it, to help those living with HIV/AIDS to continue the best course of care for themselves, help them adhere to their treatment and enable their self-sufficiency.
The Dimock Center’s Medical Case Managers and Peer Advocates help clients to navigate through our multidisciplinary clinic, as well as to outside facilities. Our Nurse Medical Managers provide medical care in the Adult Medicine Clinic and are in close contact with the MCMs and Peers. All clients also have access to support groups, educational meetings and referrals.
The comprehensive care we deliver seeks to help our Community Care patients keep their minds and bodies strong. The Adult Medicine staff works closely with the staff in the Health and Behavioral Health Centers, as well as our Child and Family Services to improve the lives of those we serve.
Specialty Clinics and Services
The Dimock Center’s Care patients benefit from services which include: medical case management, peer advocacy, nurse medical management, primary care, HIV specialty care, OB/GYN, eye care, dental, nutrition, mental health and housing services as well as an on-site pharmacy.
Integrated testing for Hepatitis C, Gonorrhea, Chlamydia, Syphilis and HIV is provided by Community Care and available at various times and locations within the Roxbury Community.
Our services are provided in English, Spanish and Haitian-Creole.
